  1. The rehab team at The Pavilion is awesome. They are all so positive and upbeat. I was wheelchair bound when I got to The Pavilion and I walked out of there with the aid of a walker. While I was in the gym doing my exercises, I watched as they worked with the elderly patients and saw the wonderful care they took with them. Anu and Lynette, who were my primary therapists, worked with me 5 days a week. They were encouraging, patient and supportive of me through this whole ordeal. I always felt safe. They never had me do any exercise that they did not believe I could do. They are true professionals who I now consider my friends.

    I was at The Pavilion for approximately 2 months. I was in a wheelchair and relied on the nurses and aids for almost everything. The nurses and aides are professional, caring, hardworking and dedicated to their patients. I want to give a special shout out to my nurses, Hilda, Gloria and Ms. B. and my aides Cynthia, Nicki, Guy and Job, who were my main caregivers that took wonderful care of me.

  2. The Pavilion at Creekwood was neglectful of my mother. They administered medicine that counteracted with her existing medicine and did not respond timely to her calls. Both of these reasons landed her back in the hospital.
    My mom called for the nurse to help her with the bathroom and they would prefer to clean her up vs. take her. Once when I visited I asked for fluids and after 20-30 mins of waiting (after having waited 40 mins for them to even respond), I visited the nurses station where they were socializing and having a great time. I asked about my mothers water (which was empty when I got there) and the nurse pointed to my moms cup and said “there it is, I haven’t gotten around to delivering it”. The nurse staff are unprofessional and rude.
    Do NOT send your loved one to this facility. A lot of abuse and neglect going on besides just my mother.

  3. The facility (Pavillion at Creekwood) did not fulfill the obligations to care for my loved one. A lot of abuse and neglect leading to severe bed sore, not feeding, offering fluids, changing and turning when due for a patient with dementia that cannot care for self and from failure to care properly. Neglect to give medication, fluids and change for someone who is bed/chair bound. Forcing food down my loved one throat when he already suffered stroke with obvious change in physical and mental status leading to aspiration when he suffered stroke in their care and did not send to ER on time until after few days when I visited and the nurse stating his condition has been like this since yesterday and when asked why she did not send to ER, she responded that she was going to be at work today and will see. I got there around midday meeting him in this critical condition and had to call 911 on my cell phone. My loved one eventually passed away. Please do not send your loved one to this facility. It is a death trap. A lot of abuse and neglect going on in this facility.

  4. I have been favorably impressed by the care and services provided in my time at the Pavillion. In particular, I am grateful for the rehabilitation staff. The team worked to allow me to recover from a broken leg. The experience was pleasant and professional.
