Communication with caregivers is very poor because there isn’t any. They never answer the phones no matter what time of day you call. And if you leave a message you will not get a call back. The CNA’s, nurses and PT’s are great though, shout outs to Nicole, Jackie, Eunice, Gladys and Audrey on the 2nd floor for their amazing care and empathy when taking care of my husband. Elaine at the nursing station, thank you, you don’t go unnoticed for your hard work! Special thanks to Nurse Manger Kim for communicating for me to others when I couldn’t my concerns.
Any upgrades being made in patients rooms are using cheap materials that break easily. The food is horrible, overcooked mushy vegetables that look like they came out of a can, hotdogs, sloppy Joe’s, hamburgers, french fries and rice and beans. This is what was served to my husband who is supposed to be on a diabetic, heart healthy diet. They had to increase his insulin because of this.
If you have to choose this facility because you have no other choice, then I advise that you be there to constantly advocate for your loved one. The three stars are for the staff only.
This place needs to get better with their communication, sterility, food/menu/food preparation for dietary needs and hiring for their understaffed facility.
Very accommodating, friendly staff, great with patience. Works with the family well. Just very nice overall.

My mother just came out of this facility. I must say, that since day ONE they failed in caring for her. My mother was first on the 2nd floor. After a week she was sent to the 3rd floor where supposedly was going to be better since she only speaks Spanish and there were more Spanish speaking staff there. Well, it wasn’t like that. My mother who has speech issues due to her condition and the language barrier made everything more difficult. But the major problem in this place are the CNA’s. They neglect the residents, they just don’t care! They are NOT doing their job! My mother was left with a soiled diaper for hours at a time. They weren’t showering her. There were times she was left without being help to eat. She needed this help. I could just go on! And all these happened even when I was there 3- 5 days a week to check up on my mother, to help feed her, etc. Things change a bit after I started to advocate for my mother, put complaints, I would call whenever I needed so my mother could be given the help or the attention she needed. I would just show up when they didn’t pick up the phone. I know they hated me there! I saw and heard many things since I spent plenty of time there.
The therapists were excellent and compassionate. The supervising nurse Danielle was helpful and always professional. The CNA’s are a lot on their cellphones. I must say I did meet some nice and helpful CNA’s such as Karen, Erminda and 2 others that I can’t spell their names.
Lashon Harrison (business office manager)was supposed to help me with my mom’s medicaid and she failed to do so. She wouldn’t give me proof that she sent her documents to SS.I requested proof now that my mom was discharge and she didn’t provide anything. Some people get paid and don’t do their job! She never returned calls, never called back when left messages. Perhaps she was told to dismiss the application because one time I attempted to have my mother out of this place before her 3 month stay. Cristel, the social worker pretty much conditioned me or threatened me by saying that if I did that, she could just dismiss the application for her medicaid since they were helping me. So, go figure what really happened…
Lastly, Don’t forget about your loved ones when they end up in places like this. Go visit them, keep an eye and advocate for them. The most vulnerable are the ones more at risk of mistreatment and neglect in facilities like this one.

I was here for Rehabilitation. They have the best therapist for a physical therapy. staff was very wonderful and also the nurse.This is the first place for a Rehabilitation.
