Bridgewater Center for Rehabilitation and Nursing is a long and short term care facility providing services to the disabled and elderly for five decades, offering a complete range of medical, nursing, and rehabilitation services.
Front desk personnel were pleasant and helpful. Room appeared clean.
Very friendly and caring staff. Administration has been very welcoming and ensure orientation/ education is focused on providing individualized care.

The patients are abused and neglected! My mother was on floor 2b. There was a man that would frequently fall out of bed, the nurses all exclaimed “I’m not going to pick him up again!” And “he can stay there and learn something.” They then left him there for 3hours on the floor! Another case of an older woman that fell in the bathroom and broke her him from the fall. Her screams of “help me, someone please.” Could be heard all down the hall. I could hear her. The nurses said “yeah she just sits around screaming. She’s fine.” She had laid on that floor for more than and hour and was taken out in an ambulance during my visit. My mother spoon fed a man that couldnt feed himself after she couldnt bare to watch any longer his trays would sit in feont of him as he stared at the food crying for 2 days of nurses refusing to feed him, his arms didnt work any longer due to stroke. The nurses often will come into a patients room to unplug the call bell then leave without helping. My mother stayed for 2 months and was denied a shower every single day of the 2 months. The food is putrid. I wouldn’t feed it to my cat, half the time I couldn’t even tell what was on the plate. It was filthy, there were 12 breakfast trays in the rec room, that sat all the way to dinner, same with the lunch trays from that day, not being cleaned. My mother and roommate had 4 trays from a previous day still on the tables in their room, not once was a floor mopped and only swept a hand ful of time. Trashes were over flowing for days. And the bed sheets were changes twice In her stay. This place is a horror show. Straight out of a torture film. ALSO. everytime I visited it reeked strongly of Marijuana! My mother and I have caught the nurses smoking weed during their shift. All the nurses were with no expecting, totally high out of their minds! With every visit all I could hear are screams and cries of patients begging for help.
This building is disgusting. It hasn’t been updated since the 20’s i swear. Even the meds are metal frame with a hand crank to lift and lower them. None of the bathrooms are handicapped accessible so majority of patients are using comodes, which are emptied only once a day. People stay for years or more have to share rooms with eachother, meaning using commode in front of eachother. They have not swept or mopped my mother’s room in the 2 weeks she’s been there. And they don’t have any drinks, water, or snacks between meals. The food it putrid, often stiff and cold. Do NOT chose this facility.
Update: 2 weeks ago my mother called me frantically and crying. I immediately picked her up and brought her home. The facility was not scheduling her important appointments for her open wounds and even canceled one of them without her consent, she had to wait 1 extra month with a large open wound for an appointment because of these people.
Nursing staff was awesome
