Word of caution. The PT administrator and assistant are not above lying to the state investigator about their practices. The administrator is rude and belligerent. No way he can resolve an issue through investigation. His teams lies are truths to him.
My friend is there and was deemed incontinent and incompetent. PT refused to properly assist him. He is now walking after a massive heart attack and 2 strokes.
Nurses, CNA’s, OT, speech therapy and everyone else has been supportive and friendly. And I have videos!!!!
It is sad that the few cause it to be a bad experience.

A friend of mine stayed here when they broke their hip. They told me the food was always cold. Some of the aids were rude like when my friend called for help. The aid would come in and some would say what do you want I am busy! Some just had a attitude that my friend was afraid to call for anything. The aids looked annoyed they had to do work. Pain pills and other pills were always late because the aids never tell the nurses that they needed the pills or nurses forgot about them. When they were able to move every time they went in the hallway there was always call lights going off. They would go down the hallway the aids and nurses and would be sitting in the nursing desk area on their phones or taking like it’s social hour. Also some of the change nurses need to go back to school my friend said because they don’t know anything. After hearing all of this I would not recommend this place.
Have to keep asking for help. Needed the bathroom, been overv30 minutes. Pl6scwanted to lay down, no one to help put him in bed because the person went on break for an hour. Grrrrrr
Short of staff daily. The staff there is overworked and do not care. The ladies up front are good salespeople. Don’t believe them.
My grandfather received excellent care here and recovered quickly after having a stroke. He was able to regain use of his legs and now is self sufficient with the use of a walker. Thank you to the awesome therapy team at Forum!
