My father was transferred to Hemet Hills Post Acute on Friday evening11/8/24. He was okay yesterday 11/9/24, talking and wide awake and alert…I stayed at the facility until about 9:30 last night. Today he seems to be declining and extremely lethargic. When I arrived today my father was laying flat in the bed and when you have a Gtube you should not lay flat as the patient could aspirate. When the charge nurse finally came in the room she didn’t know what was going on with the patient and did not know the patient history. All she knew was that the nurse she relieved said that was my father’s baseline was unable to talk and hard to wake up and that is NOT his baseline. All she knows was that he was there for muscle waste and diabetes. Charge nurse said that because its the weekend medical records is not there so they do not have any information and that is completely unacceptable! My father is recovering from a stroke, had a heart stint, high blood pressure, MRSA in his septum, recovering from pneumonia. If the facility did not receive the medical history from transportation then the transferring hospital should have been contacted to request a duplicate copy. He has not been getting turned every few hours to prevent pressure sores. My father doesn’t have on SCD boots or TED socks to help with his circulation. Staff unable to give updates. My stepmom calls and can never get a charge nurse on the phone. He is a fall risk and he is in a bed with no rails, he needs a bed with an alarm. He has terrible shoulder pain which has worsened severely since being at Hemet Hills. The room hadn’t been cleaned from the patient prior to him as the previous patients clothes were still in the drawer, the drawer was broken, there was no remote for the TV and the staff was unable to locate one or a remedy so I brought a universal remote in today when I visited so he could watch TV. The room was so hot when I got there today.
