We have been providing our community with outstanding nursing, rehabilitation services and extended care for over two decades. Our staff of trained professionals are dedicated to providing a professional, nurturing and respectful environment. We specialize in post joint replacement therapy, stroke recovery and complex wound care.
We are an 87-bed, skilled post acute care facility located at the corridor of Pleasant Hill, Martinez and Concord.
Facility appears to meet standards, however care & service can vary widely depending on staff. Staff does not inspire much confidence and I would look elsewhere, even much farther away, before considering this place again.
My father had hip surgery and needed to get stronger, so he can return home as my wife and I work and are unable to help during the day. They had the best staff from administration, to nurses, best therapy staff, cnas, and he loved the food which he’s a pretty picky guy. I would recommend this place for my loved one.
My 87 year old father was recently at Legacy as he required wound care after having an intensive surgery. It was his first experience in a skilled nursing facility after being in the hospital. The staff there were WONDERFUL!! Cindy, the director of nursing, the LVNs, RNs, CNAs, and PT all did a wonderful job of helping him heal and build back his strength. Not only did they take great care of my father, but the staff always made time to answer my questions . I was there every day and house keeping did a great job of keeping the facility clean, and my father was pleasantly surprised at the quality of the meals the kitchen staff provided. I would highly recommend Legacy Post Acute.
My husband was sent here for physical therapy. I highly recommend this place. I would not let him go any other place if ever had to go for a rehabilitation again this place was fantastic and the staff was fantastic. My husband felt so comfortable there. He said that the nurses and administrators were so caring and supportive.
When I first arrived I saw a sign that read, “A Commitment to Caring”. At first I thought it was just a saying and nothing more. We decided to take a chance and enroll our eighty (80) year old Mom, Dr. Yi. After three (3) weeks we walked out today. I saw the same sign only this time I had a different opinion of it. The staff at Legacy Nursing and Rehab really do have a commitment to caring. Our Mom’s needs were met every step of the way. Time after time she pushed the infamous white button an either an RN or CNA responded right away. I’d like to personally thank the following people. First there is Christine the receptionist. She’s a sweetheart. Then Stephanie who is the Administrator. She accommodated us when we asked for a different bed. Then there are “the Saints”: Sharletta, Jean, JoJo and especially Velma. Of course there were the CNA’s whose names escape me and Dr. Allen who as with Mom every step of the way. All I can say is THANK YOU ALL and GOD BLESS YOU. Oh yeah, I almost forgot. I highly recommend Legacy Nursing and Rehab to everyone!!!!!!!!!!!!!!!!!!!!!!!!!!!
