If your loved one is living at home and may benefit from nursing, medical and rehabilitation services, while providing socialization during the day, Middle Island Adult Day Health Services is here to help. Our Adult Day Health Services is a medical model day program providing medical care and socialization to those living at home, while also offering respite to their caregivers. Working with your community doctors, our program manages all the health and medical needs of each participant. Our program is monitored by registered nurses in a day setting, allowing these individuals to remain at home and in their communities. In addition, we will assist with coordination of services which allows families and caregivers to maintain their daily schedules.
