  1. Important thing to note, most of the reviews are from employees, former employees (the 5 star reviews) and residents at the facility (the 1 star reviews). My review is completely as an outside observer looking in. First I would like to point out that there is a massive “shortage” of employees particularly at this facility. Probably because no one wants to work here under such wildly incompetent management. I have watched employees stand around bloviating about their day in the hallways, while residents at the facility sit in their wheelchairs covered in their own filth waiting for hours patiently to be lifted up with a crane to be placed in bed. I have watched employees walk into a room with the “I need help” light on and simply turn the light off and leave. I have watched the people you love and care about be forcefully neglected by the staff because it’s more important to maximize the flow of insurance dollars than do their job or hire enough people to maintain the minimum amount of required nurses to operate the machines to assist the elderly. After reflecting on my initial review I realize that my attempt at comedic relief was simply making a serious situation appear less serious. I want to be straightforward about this facility so you know exactly what to expect if you choose to bring anyone you love or care about here.

    1. Neglect: expect to be ignored and just know, the staff doesn’t care.Is your loved one drowning in their own vomit? I hope your loved one knows how to operate their bed because if not the staff will probably just turn the emergency light off and walk away. (like they did to mine)

    2. Fire concerns: They don’t have enough nurses to operate the machines to get your loved one in and out of bed (it requires 2 people) so if a fire breaks out feel free to bring an urn to collect the ashes.

    3. Nurses, aids, assistants, PT staff: every time I walk out and look around they are on their phones, yakking it up in the hallways, at the nurse station pretending to work. Also don’t you dare take photos or anything at all while you’re here, transparency is a huge “no no” the social worker will come running as fast as possible to threaten you with a discharge plan if you dare to take pictures or video of anything that will make them look bad.

    4. Not so 24/7 facility: if it’s past 6pm good luck getting in. you can stand outside for hours until someone comes to open the door.

    5. Insurance fraud: insurance is paying anywhere from 5-10k to rehabilitate a person and get them home. From my own experience, none of the people here are receiving enough care to ever get them home. They want to shuffle your loved one from rehab to nursing so they can cash in and get paid to ignore your loved one some more.

    6. A facility that specializes in Alzheimer’s patients is like a sex offender specializing in vulnerable women with amnesia… nothing here is noted, none of the staff can answer any questions about anything. But if you point out their failures expect them to threaten you with “discharging plans” that’s right, even though you’re paying them to do a job they refuse to do. well… somehow that’s your fault i guess… you should have had the psychic awareness to know this place was terrible before even coming here…

    7. Why are we paying you if we still have to show up daily to keep our loved one alive? I would love to know the answer to this. I would like to know why insurance isn’t paying me 5-10k a month to take care of my own loved one. If I neglected my loved one the way this facility has I would be in prison but because insurance shells out the money for it, I guess it’s perfectly legal to just watch my loved one to die from starvation in a “rehab” facility. It’s weird how someone eats and smiles at the hospital until they get to Sundance and suddenly it’s 3 weeks without food…

    This place needs to close its doors and send their patients to a better facility. One that treats people with some level of basic dignity.

  2. I don’t know where to begin. I guess I’ll start with the positive which is the handful of staff who bust their butts taking care of patients (all 3 weekend nurses, Ashton, Patricia, Nancy, Michelle, Elaine, Sandy, the 2 Kayla’s (a nurse and a cna), Mel, and the rehab department. If it were not for these people, I would have transferred out mid December.

    The top 2 complaints going back through reviews are still a problem to date (my discharge date was 1/27/25). There is a severe staffing issue, seriously severe, 1 or 2 CNA’s on Monday, Wednesday, Friday expected to cover 160 patients. Elaine or Sandy often times put in that position. And yes, other staff chips in to help with some things like getting food trays out. Those high up certainly are not doing brief changes, clothing changes, or getting people out of bed. There are other days the staffing is poor randomly. And the facility is always short on supplies running out of gloves, paper towels for the bathrooms, and if all things briefs on 2 occasions.

    The next important mention is the food, something brought up consistently in other reviews going back 2 years ago. It is always cold and many occasions meat is not fully cooked. Technically, if they haven’t credited my meals as I rarely ate any food, they are committing Medicare fraud. If Medicare is billed for food I didn’t eat-FRAUD; everyone there, staff and patients, are well aware I sent my trays back to the kitchen untouched.

    There is a staff member who is still working there who denied me access to my walker which is needed for my transfers. Instead she had me use a nightstand which nearly caused a fall.

    There is another staff member in a position of authority who is aggressive, does an exceptionally poor job ordering supplies, plays favorites with staff, and is rude in general (this person has ignored family members in the hall who say Hi).

    People are left in urine and crap overnight due staffing issues. My last month there, I went 8 days without a shower twice. My roommate was there for exactly 2 weeks and 1 day, she had 1 shower. The long term side constantly smells like pee, people are calling out as that side tends to have staffing issues regularly.

    When issues with staff are brought to their attention, they simply move that individual to the side of the facility where the people cannot speak for themselves-real classy. The corporation Cantex that owns this facility cares more about profit the your family members, period. Food service gloves are the medical gloves staff use, food is delivered by US Foods and it’s the cheapest food is ordered (the Mac and cheese tasted like masking tape-seriously, how is it possible to screw up Mac and cheese? They managed to screw it really well).

    If you need anything when they are handing out trays, you gonna wait so hopefully it isn’t an emergency. This is really bad on short staff days so it occurs often.

    I could go on and on, it’s terrible with the exception of some amazing staff I had the privilege to meet. I was admitted on 10/20/24 and it was decent, except for the food. Once the first week of December arrived, it was a steady decline to the bottom. If you place your loved one here, they won’t be eating well or good healthy food, they might have good staff depending on what hall and the day, or possibly restricted from using equipment they need for mobility.

    Cantex, keep your generic auto investigation reply to yourself because if you cared, these several year old issues wouldn’t be issues currently in your facility.

  3. My mom was admitted to Sundance for rehabilitation after a broken pelvis from a fall. We originally asked that she be admitted to another facility but that didn’t happen. Once we knew she was going to Sundance, we read reviews and got concerned. We are thankful, that we didn’t go off the bad reviews and gave it a try. Overall, the care she received was wonderful. She was happy with the food. We are happy she is up and walking again. Although, everyone took excellent care of her, she wanted to especially thank Elaine, the CNA and her therapists, Tyler and Roland. Hopefully, she won’t have to return but we wouldn’t hesitate if she did.
