Our team of clinical and medical professionals work closely with patients to design personalized care plans focused on a safe, expedient recovery in a setting that promotes comfort and dignity.
Located near the hospital district of downtown Sugar Land, Texas, The Crescent is a welcoming 112-bed skilled nursing facility that cares for patients who are recovering from a serious illness and those who require advanced therapy services after a hospital stay.

The staff was very rude, uncaring, will not answer room call. DO NOT send your loved ones there. Go somewhere else.
Don’t listen to the dishonest person during your tour of the facility; trying to get you to choose this location to put your loved one in because they will sell you a book of promises that sound good and then once your loved one is there, the promises are forgotten.
This is the absolute worst place to put your loved in, so do yourself a favor and just skip this place and do the research and find a better facility for your loved one to rehab and get stronger to go back and live the life they had before needing to be rehabilitated.
On his arrival in the late evening, no one called his daughter to give her a formal written plan for his care and treatment to get him rehabilitated.
I visit him shortly after he arrived to this place and opened my Brother’s drawer to check to see what it looks like on the inside and there is a roach running on the edge. That is definitely a red flag to me that there is a roach problem there as well.
My brothers 1st day there (not even 24 hours) and he has fallen out of his bed anc left on the floor to be discovered when nurse made rounds. Nurse called my niece ar 5am to inform her with no details. When she tried calling later over 5 times with no answer throughout the day and trying to get an update on her dad. At the end of the day, she found out he was taken to dialysis but was not informed. The family was worried all day because no one could reach my brother or knew what his status was. This is what I mean by no one my niece a plan of care. My brother had dialysis on Monday before his arrival to Crescent and I was not aware you could have dialysis 2 days in a row,
Communication from medical staff to families is definitely lacking and Cresent needs family advocates and ombudsman in place for families.
Clearly, there is some type of issue when you call up there and get transferred to the nurses station 10 times during the day and not one single time you reach a nurse. No calls are answered from nurses station. You cannot tell me that you are that busy throughout the day that not one nurse can answer the phone. This is unacceptable care for the cost paid to have a loved there.
This experience has also opened my eyes to the medical care community as a whole and inspires me to really inquire about nursing school and opening up my own facility one day to really show empathy and proper care to the patients and the their families.
No family member or patient should have to go through this and I even asked the receptionist how would she feel if this was her dad or her uncle or her brother that was in this situation?
MY MOTHER WAS AT THE CRESCENT FOR A LITTLE OVER 3 MONTHS AND SHE RECENTLY PASSED AWAY AT THIS FACILITY FEBRUARY 6TH 2025.. MY MOTHER HAD BREAST CANCER SO MOVING AROUND HAD BECOME EXTREMELY DIFFICULT FOR HER. THEY DID THEIR BEST TO TRY AND GET HER BODY MOVING AGAIN BUT WERE UNABLE TO GET HER MOBILE AGAIN THE CANCER WAS JUST TO FAR ADVANCED. SHE ENDED UP BED RIDDEN AT THAT POINT WE HAD TO BUT HER ON HOSPICE CARE AS A ADDITIONAL CARE AND IN ADDITION TO THE NURSING SERVICES SHE WAS GETTING AT THE CRESCENT. IN MY HONEST OPINION IS THEY ARE A LITTLE UNDER STAFFED BUT THEY REALLY DO CARE FOR THE PATIENT AND DO THE ABSOLUTE BEST WITH WHAT THEY HAVE. THE FACILITY IS VERY CLEAN AND THE MEALS COME ON TIME EVERYDAY. THE QUALITY OF FOOD IS ON PAR WITH HOSPITAL FOOD MAYBE A LITTLE BETTER. THE NURSING STAFF IS QUALITY AND THEY ARE GOOD PEOPLE. MY MOTHERS OVERALL EXPERIENCE WAS A GOOD ONE. WE WERE FORTUNATE FOR THIS FACILITY THE CRESCENT TO BE ABOUT 3 MINUTES FROM MY HOME IN SUGAR-LAND-TX SO IT REALLY WORKED OUT GREAT. I COULDN’T HAVE ASK FOR A BETTER SITUATION OR CARE FOR MY MOTHER AT THE CRESCENT. THANK YOU CRESCENT, AND ELYSIAN HOSPICE, FOR THE BEST CARE POSSIBLE FOR MY MOTHER. 1
Great place to be, friendly staff. Will encourage you to bring your loved one here
One of the best jobs I ever had……we are family and we are a team……one band one sound!!!!!!!
