Horrible experience! Response time when assistance requested often exceeded 45 minutes. Staff rude to patients. Dad fell 3 times in 6 days due to staff negligence. Misrepresented medications given, video to prove it. Never ever received a fall report. Staff totally disrespectful.
My mom, eho is just shy on 90 yrs old, went to this place for rehabilitation When she arrived she could walk using a cane, but was on fall prevention. Even though this place had tons on walkers she along with 99% of patients I saw in hallways were given wheelchairs to move about, while multiple orderlies and medical personnel walked about. After approximately 2 and a half months of this I had her discharged. When I and a friend picked her up SHE COULDN’T EVEN STAND BY HERSELF EVEN WITH ASSISTANCE. Now HALLELUJAH here we are 3 weeks later and she is walking around our home with a walker and still growling strong.
The most DISTURBING part of all is that the physical therapist WHO COMES TO OUR HOME 2x per Week, used to work at Legend Oaks during my mom’s First 6 weeks at Legend Does NOT REMEMBER EVER SEEING MY MOM IN THE PHYSICAL THERAPY ROOM there. In Legend Oaks defense I was told by them that in mid February mom “said” she didn’t want to do physical therapy, Her story is even longer than this, but hopefully the information I did share is enough to help anyone considering this place to go in and look around Legend Oaks more in-depth than I did.
My play mother went there for respite care on Wednesday, February 19 only to be there for four days transit came and picked her up safely and she was supposed to come back home on Sunday the 23rd by 9am. She had several visitors while she was there those few days and even a visit after 4 PM on Saturday, February 22. Again, she was supposed to be home by 9 AM on Sunday. We did not receive a call that she was running late or anything in regards to her transportation. My sister had received a call at 6 AM but was missed and we were not able to reach them again when mom pulled up. It was a horrific site face was battered and bruised lip busted with a black eye and swollen cheek. Four days later she is still recovering and in pain I don’t know what is going on up there or even if they wrote up an incident report, but we need answers.

My grandmother was at Legend Oaks from February 19 to 23, 2025, for respite care, and the condition she returned home in was absolutely unacceptable—her face was swollen, her lip was busted, and she could barely walk. When I asked my Aunt how she felt, she said terrible, and seeing the tears in her eyes devastated me. Family members who visited reported that the staff were rude, dismissive, and downright offensive. Shockingly, my 86-year-old Grandmother, who is on hospice care, was severely neglected during her four-day stay. I would strongly advise against choosing this facility for anyone in need of proper care.
We did our research before making this choice; their four-star rating and seemingly pristine facility led us to believe we were selecting a safe environment for my Grandmother. However, that was far from the reality. Additionally, my aunt called to inquire about what happened to her mom and was left on hold for over 40 minutes before the call was disconnected.
Do not send your loved ones here! This is outright abuse, and the staff here will ruin your experience.
