Trabuco Hills Post-Acute is a skilled nursing facility located in the heart of Lake Forest, California. Our facility provides compassionate, skilled nursing care 24 hours a day.
Our live in full-time care giver for my Dad was not impressed with the attention to detail here. We transfered my Dad to a different place immediately. I had great ease in dealing with Liz the Social Worker but, She can’t do everyone’s job for them to make it better unfortunately.
Don’t walk, run away from this place. They failed to get my mom’s UTI results in a timely manner. When they did finally receive them, it was only 2 days after they got the results that they gave her antibiotics and only because I asked about the test results. I got a different story every day when I asked if the results were back. The test was done, the test wasn’t done, etc. The aides did nothing to assist my mom eating. They’d ask her if she was done and she wasn’t responding. So they would just remove the food. If I hadn’t been there to feed her she wouldn’t have eaten for several days. Astonishing negligence.

Remember that this is a post injury rehab and not a resort. With that in mind, this facility exceeds most others in the area as word of mouth from other patients shared testimonies . Large staff, great PT and OT, I don’t eat much right now, but the food is very good and huge portions. Everyone else loves the meals.
Daily activities include games and interactive activities. Music afternoons and ice cream Fridays. A hairdresser
Is available on Wednesdays by appointment, etc. I definitely recommend this facility.
Trabuco Hills Post Acute is an incredible facility for post acute rehabilitation. My mother Carol Rigg arrived from Mission Hospital after a stroke. My sister and I chose the facility for the incredible PT, OT, and rehabilitation. The PT are incredibly trained, the facility is clean, and they work miracles of rehab. My mom could not swallow, walk, sit-up and her left side didn’t move. Today she eats all foods, speaks, sits up, transfers to wheel chair, is walking again with aid, feeds herself, is engaged in life, and has her same personality. BTW, there is a nice outdoor garden and mom eats outside or visits as do the other patients. It takes time to recover and physical therapy work by the patient to recover.
