The new Trinity Elms Health & Rehab center is designed for long-term, skilled nursing care as well as short-term rehabilitation, incorporating a homelike design with personalized resident-centered care.
Trinity Elms Health & Rehab, which opened in July 2014, sits adjacent to Trinity Elms Assisted Living. It offers four small neighborhoods designed to meet short-stay or residential needs.

While I agree that they are short staffed, I don’t think this is unique to Trinity Elms. In general, the staff is helpful, caring and has a genuine interest is seeing that all patients are well cared for.
The facility is clean but there is a lack of communication between all staff. Contacted the Director of Nursing and she acted if I was wasting her time. She referred me to her assistant which was very helpful. My Mother got bedsores while a patient there and no one contacted me. I found out myself. My Mother wasn’t changed often. No one encourage her to eat or participate in therapy. She wasn’t bathe every day. Half of the staff wasn’t able to provide me with information. No daily chart to review updates or progress. Ask questions of the visitors before you make a decision.
Place could use some work with staff attitudes and things of that nature. There are some really great hard working staff who works here. I wish they were more attentive to each individual patient but everything seems clean for the most part and patients are wonderful.

My father was sent to Trinity Elms from Davie Baptist for Rehabilitation for strength rehabilitation…. stayed a lil over 3 weeks…. we had to remove my father due to the non care and decline of his health in this facility… they basically had my Dad in his bed… rehabbed him an hour a day… took his personal items that my brother found at the nurses station (candies etc.) he literally had 1 shower in 3 weeks… numerous calls were placed for complaints and never returned any of our calls but emailed us .. my dad left Trinity elms and was taken to Davie Rehab center in mocksville where they basically had my dad walking in 2 weeks and showered daily… trinity elms at Christmas time had heat broke and no water in the building for 3 days… i took these photos of my dads room on the other side of the extra bed of a dirty pan and garbage that sat for days till i mentioned it to the nurse…. when i picked my dad up to go to Davie Rehab in mocksville , he was missing his personal items… when they moved my dad to another room due to no water or heat,, they failed to move his belongings with him… i walked the whole rehab facility and found my dads belongings ( hearing aids, clothes, hygene stuff) in a room on the other side of the building that was uncleaned and all his left over stuff still there including a half a soda left for days from him in a cup… they had his personal walker locked up in a storage room that took several employees time to find… they said it might be in the rehab room… im like why does my dad have his walker in a rehab room where they should have their own rehab walkers.. i hope you read this and keep in mind if your loves one is in need of rehabilitation… this is not the place for them… please re evaluate your family member in this establishment..
