Since opening in 2003, PTC serves as a post hospital resource for patients without a safe place to heal from injury, illness, or another medical setback. Our 34-bed program is an excellent discharge solution for health care providers. We provide services to approximately 200 residents annually.
Pathways to Care (PTC) is a not for profit, licensed Assisted Living Facility, which provides medical respite to people experiencing homelessness.
Pathways has truly been a blessing to me. I was referred here to recover from a serious medical condition upon discharge from the hospital, as I was homeless and required daily iv antibiotics. During my hospital stay my car was towed and impounded so I lost my car along with all my clothes and possessions. Pathways gave me clothes, a laptop, and a bicycle to help me get back on my feet. The staff truly cares about you and does everything in their power to help you succeed. Thanks to Pathways I have my sobriety and my health back.
